Chronic Condition Management

The postgraduate programs in chronic condition management aim to meet the extended professional development needs of graduates involved in policy, planning and/or delivery of services to people with or at risk of chronic physical and/or mental health conditions.

Core principles and skills involved in developing and implementing self-management enhancing programs in diverse settings for health professionals and service users are central to the programs. They address the broader issues around chronic condition management and self-management implementation within services and individual practice.

The topics accommodate participants’ individual areas of interest and varied roles, and provide for learning across management of a diverse range of chronic conditions.

What you will study

These courses seek to offer you an understanding of the core principles and skills involved in developing and implementing self-management enhancing programs in diverse settings for health professionals and service users. In particular, they will help you understand the issues around chronic condition management and self-management more broadly and how to implement these initiatives into your service and individual practice. Topics cover areas such as chronic conditions management for specific conditions, contexts or populations; client-centred health behaviour change; self-management support approaches; implementing change in healthcare systems supporting chronic condition management; and the person’s experience of self-management.

Graduates are highly valued by employers for their specialised knowledge in chronic condition management. There are increasing opportunities for graduates in a wide range of health and policy related areas. Many become dedicated chronic disease coordinators or move into more focussed project and service roles.
